Position Summary

Key Accountabilities and Duties of a Support Worker

  • Support clients in participating in age-appropriate activities and tasks, including employment, in-home support, and community access.
  • Support clients in accessing activities and outings in the community.
  • Provide direct care assistance to each client in accordance with their service plan and as directed.
  • Work in accordance with the medication administration policy, work health and safety procedures and client rights, including privacy and confidentiality.
  • Monitor the safety and well-being of the clients and report any concerns to the Manager.
  • Provide 1:1 or small group training and support in response to individual client needs.
  • Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication books as required.
  • Participate in the support planning process for individual clients and monitor outcomes.
  • Work with clients, their families/carers, relevant case managers, and other professionals to support the clients’ individual goals.
  • In conjunction with other staff members, assist in developing, implementing, and evaluating training and behaviour programs to attain identified individual goals.
  • Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
  • Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
  • Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.
  • Report regularly to their line manager.
  • Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
  • Provide backup and support to other staff as necessary.
  • Participate in Performance Development meetings to identify ongoing training and support opportunities.
  • Notify the service if unable to work rostered shifts, with as much notice as possible.
  • Clients receive appropriate levels of support essential to promote and maintain independence and participate in age-appropriate activities.
  • Ensure all clients receive a high level of care and support.
  • Provide personal care to clients in a caring and supportive way.
  • Demonstrate service procedures are followed with documented evidence.
  • Comply with all internal policies and procedures.
  • Ensure accurate records that demonstrate clients have received correct medical and dietary intakes as prescribed by medical practitioners and as per service policy and procedures.
  • Maintain a high level of communication with the Director , including advising of any potential risks to clients or the organisation.
  • Comply with mandatory training in WHS, Manual Handling, Medication and Infection Control every year.
  • Comply with the NDIS Code of Conduct to support participants in ways that are respectful and inclusive.
  • Coach and build confidence in participants to develop the skills they need, such as cooking, budgeting, and shopping.
  • Provide personal care, such as showering, dressing or other tasks, responding flexibly to participant preferences.

Qualifications, Skills and Experience

Qualifications:

  • Minimum Certificate III in Disability or Individual Support.

 

Experience:

  • Experience in supporting people with disability.
  • Experience in manual handling (desirable).

Other Requirements

  • Current Working with Children Check.
  • First Aid Certificate or willing to obtain one.
  • CPR Certificate or willing to obtain one.
  • Manual Handling Certificate or willing to obtain one.
  • Medication Administration Certificate or willing to obtain one.
  • Immunisation evidence (COVID-19 and Flu).
  • Current Driver’s Licence and the ability to travel locally.
  • Compulsory third-party insurance on private vehicle (to be used for work purposes).
  • Right to Work in Australia.
